Goals & Outcomes
Generally, the nutrition consultation provided by a Registered Dietitian is most useful in helping individuals help themselves or improve their nutrition. You determine the nature and amount of change you wish to make.
Confidentiality
To maintain a level of professional integrity Dietitians are required to consult with other health professionals on an ongoing basis. At Marsh-Knickle and Associates, all independent practitioners meet regularly to discuss individual cases to maintain a best care practice. We understand that the information you share is of a personal nature, and that you would want it to remain private. Confidentiality will be maintained unless you give us specific, written permission to share information with another person or agency.
Payment for Services
Unless otherwise stated on this form, our fees for nutrition services are
$90.00 for 60 - minutes initial assessment
$85.00 for 60 - minutes comprehensive initial follow up
$100.00 for meal plan with resources
$80.00 for regular 60 - minutes follow up
$50.00 for 1/2 hour Follow up sessions.You, the client, is responsible to make payment to your dietitian at the end of each session. Cheque, debit card, Mastercard and Visa are accepted. A receipt will be issued to you. Our fees qualify as medical expenses fro income tax purposes. Some extended health care insurance plans may reimburse you for part or all if our fees. It is you responsibility to determine what your coverage is.
Cancellation of Appointments
As a courtesy to you dietitian, please notify us 24 hours before your appointment if you cannot keep it. (Please call by Friday noon to cancel Mondays appointment.) Except in emergencies, we will bill you at the rate shown on our Client Information Form & Fees
